It's easy!

1. Download the approriate Adobe® PDF form below. If you have the Adobe® PDF plugin installed, the form will open in new browser window or tab. Otherwise, download the form to your hard drive and open it in Adobe® Reader®.
2. Electronically fill out the form in Adobe® Reader®. Note: you will not be able to save your electronic entries in Adobe® Reader®.
3. Print out the form after filling it out completely. Alternatively, you may print a *blank* form and fill it manually.
4. Get the completed service request to Maribel Medina on the 3rd Floor, Towers one of four ways:
   • Hand-delivery.
   • FAX the completed form to (559) 497-3809.
   • Deliver the completed form via FCOE Truck Mail.
